Our Client was taking long-term anticoagulants for a number of years as they had previous deep vein thrombosis. They were admitted to hospital with an unrelated illness and on admission blood tests showed that their INR (how quickly the blood clots) was not ideal. Therefore, the ward doctors decided to increase the amount of warfarin prescribed and to also add in other anticoagulant medication.
A few days later they were noted to have suffered a large bleed into the chest.
The excessive use of anticoagulants was a breach of duty of care and caused the bleed.
